Before the age of 18, did any of the following happen to you?

  1. A parent or other adult in the household often swore at you, insulted you, put you down, humiliated you, or acted in a way that made you afraid you might be physically hurt.

  2. A parent or other adult in the household often pushed, grabbed, slapped, or threw something at you; or ever hit you so hard that you had marks or were injured.

  3. An adult or person at least 5 years older than you ever touched or fondled you or had you touch their body in a sexual way; or tried to or actually had oral, anal, or vaginal intercourse with you.

  4. You often felt that no one in your family loved you or thought you were important or special; or that your family didn't look out for each other, feel close to each other, or support each other.

  5. You often felt that you didn't have enough to eat, had to wear dirty clothes, and had no one to protect you; or your parents were too drunk or high to take care of you or take you to the doctor if you needed it.

  6. Your parents were ever separated or divorced.

  7. Your mother or stepmother was sometimes or often pushed, grabbed, slapped, kicked, bitten, or hit with a fist or hard object; or was ever repeatedly hit for at least a few minutes; or threatened or hurt with a knife or gun.

  8. You lived with anyone who was a problem drinker, alcoholic, or who used street drugs.

  9. A household member was depressed or mentally ill, or attempted suicide.

  10. A household member went to prison.
