Modifiers

  • 95Synchronous telehealth (audio + video). Real-time audio-video. Pair with the telehealth POS.
  • 93Synchronous audio-only telehealth. Phone sessions where the payer allows audio-only.
  • 25Significant, separately identifiable E/M. Same-day E/M plus another service (prescribers).
  • 59Distinct procedural service. Two services not normally reported together but appropriate here.
  • HOMaster's-degree level of clinician. Some Medicaid programs require licensure-level modifiers (HO, HN, etc.).
  • HJEmployee-assistance program. EAP-funded sessions, per payer.
