The Alzheimer's Association Mountain Lakes Chapter covers New Hampshire and Vermont, offering a 24/7 helpline, caregiver support groups, memory screening referrals, and education programs for individuals with dementia and their families. Services include the 24/7 Helpline (800-272-3900), Caregiver Support Groups meeting across NH, an online Community Resource Finder, and care consultation with licensed social workers. All services are free. The chapter also connects families to the NH-specific dementia care network through NH DHHS and local Area Agencies on Aging.

Easter Seals New Hampshire operates Adult Day Health programs at multiple sites across NH, serving older adults and adults with physical and cognitive disabilities who need daytime support and supervision. Programs provide social engagement, cognitive activities, health monitoring, personal care assistance, and caregiver respite in a structured therapeutic environment. Adult Day Health is particularly valuable for individuals with early-to-mid-stage dementia and their family caregivers. Accepts NH Medicaid (through HCBS waiver), long-term care insurance, and private pay; sliding scale available.

Community mental health enhancement organization connecting older NH adults living with mental illness to their regional community mental health centers. Supports access to therapy, medication management, case management, and community programs for seniors across all NH regions.

Statewide coalition improving mental health access for NH adults 60+. Coordinates the Behavioral Health in Aging initiative, trains primary care providers in late-life depression screening, and connects older adults to mental health resources. Advocacy and referral, not direct services.

NH's statewide network of ADRCs, operated through DHHS, serving as a single point of entry connecting older adults and adults with disabilities to mental health supports, Medicaid waiver programs, caregiver support, in-home services, and community resources. ADRCs exist in each NH region.

The NH Family Caregiver Support Program, administered by the Bureau of Elderly and Adult Services (BEAS) through local Area Agencies on Aging, provides free services to unpaid family caregivers of adults 60+ and grandparents/relative caregivers of children. Services include caregiver counseling, respite care, support groups, information and referral, and supplemental services. Access the program through your regional Area Agency on Aging or by calling the NH Senior Helpline at 1-800-351-1888. Particularly valuable for caregivers of individuals with dementia, stroke, or serious mental illness.

NH program within the DHHS Division of Elderly and Adult Services helping older adults understand Medicare and Medicaid, protect against fraud, and navigate behavioral health benefits. SMP volunteers assist seniors statewide with benefits questions and care coordination.

Dartmouth Health (formerly Dartmouth-Hitchcock) operates a comprehensive Memory Disorders Program in Lebanon, NH providing evaluation, diagnosis, and management of Alzheimer's disease and other dementias. The program includes neuropsychological testing, geriatric psychiatry consultation, neuroimaging, and care coordination for patients and families. Dartmouth's Memory Disorders team also supports clinicians statewide through referral and consultation. Accepts Medicare, Medicaid, and most major insurance.
